Po starcie do lotu szkolnego z instruktorem i uczniem-pilotem na pokładzie, z powodu problemu technicznego, prawdopodobnie uszkodzenia łopaty wirnika, PIC wykonał dowrót do lotniska. Samolot wylądował około 300 metrów od RWY ulegając znacznym uszkodzeniom. Instruktor odniósł poważne obrażenia.
