Sprawa PKBWL 2011-1104 — 2011-08-28. Statek powietrzny: Spadochron MAMBA. Przyczyna według ustaleń komisji: Zablokowanie uchwytu sterowniczego na tylnej prawej taśmie nośnej czaszy głównej przez niesymetrycznie zsunięty slajder. Spowodowało to szybkie obroty czaszy. Skoczek wyczepił czaszę główną i otworzył czaszę zapasową.
